You'll enjoy traveling in the community to meet your assigned patients. Coordinating their care involves assessing conditions, facilitating care coordination with other healthcare providers or facilitating hospice care. Building relationships with family members and educating them on prevention methods, insurance benefits and other issues are just a few critical aspects of these positions. Many Clinical Care Managers are based out of a home office, and the position offers them opportunity to plan their own schedules within a 40-hour work week. Bilingual English/Spanish or other language fluency is especially helpful in these positions.
If you're a team player with strong communication skills, the variety of challenges presented in our telephonic care management roles could be the ideal fit for you. Because the service centers are open during extended hours, we offer a variety of work schedules that can suit your home-life needs. Evercare telephonic nurse positions can include one or more of the following responsibilities: developing patient-centered plans of care, coordinating patient care plans with community resources, serving as a patient advocate, conducting surveys and providing care management expertise, benefits determination, clinical coverage review/pre-service determination and in-patient review of hospital records. Bilingual English/Spanish or other language fluency is a definite plus in these positions.
In these management roles, you will supervise interdisciplinary clinical teams and manage all aspects of patient care while assuring continuity and quality of care. We will also depend on you to draw upon your analytical, facilitation and teaching skills to review clinical records, train new hires, mentor staff and effectively address customer service issues.
