Our Nurse Practitioners provide primary, hands-on care, as well as case management. As mobile caregivers, our NPs serve enrollees in local nursing homes, working with doctors, nursing home staff and patients' families to achieve optimal care. This is your chance to have your own caseload, primarily work regular business hours (on-call policy varies by location) and work independently but with the support of physicians and other Nurse Practitioners. Your caring hand and sharp mind will be put to work as you deliver high-quality care to Evercare members. With the patients' needs in mind, you will evaluate the effectiveness of the plan of care and make revisions, manage behavioral issues, perform comprehensive assessments, and document findings and assess the medical necessity of ancillary services.

Your combination of care and clinical experience will serve you well in this position. In the Clinical Advisor role, you will manage approximately a 50%-75% caseload, provide primary care in collaboration with assigned physicians, evaluate effectiveness of the plan of care and make revisions as needed. Your other primary responsibilities will be to foster and develop a culture of clinical expertise, oversee orientation and clinical staff development programs and promote organizational goals into specific objectives for clinical staff.
If you're an NP, a clinical expert and a mentor, you could be a Clinical Team Leader. Some of the responsibilities of this position include managing a 25%-40% caseload and delivering cost-effective, quality care. The majority of your time will be used translating organizational goals into objectives for clinical staff, analyzing and streamlining workload, supporting career growth for clinical staff, clinically orienting new hires and promoting collaboration and teamwork among clinical staff.
This is a collaborative leadership role ideal for NPs with management experience in a clinical setting and solid knowledge of long-term care and geriatrics. In this position, you will supervise and mentor NPs, manage day-to-day clinical operations and oversee the practices of primary care provider teams. You will be charged with building self-directed work teams, translating business imperatives into clinical priorities, planning site-specific program rollouts and participating in cross-functional teams.
Accept this role and take on a new level of leadership and responsibility. Utilizing the principles of continuous quality improvement, you will provide leadership, administrative support, strategic planning and overall direction of the Health Services function at a highly complex site. The success of the site's Health Services programs will be your responsibility, as will promoting a positive public image and managing a team of field-based clinicians and clinician managers.
