Community Care Manager Position

Care Managers in Evercare work within an interdisciplinary care team to develop and implement specialized plans to meet our chronically ill and elderly members' health care and social needs. Care Managers provide ongoing telephonic or personal care management to members residing in the community, assist caregivers and use other community support agencies to help see that ongoing as well as future needs are met. If you're a great listener and communicator, you are an ideal candidate for an Evercare Community Care Manager position. A Bachelor's degree in Social Work or other healthcare or related experience is required. English/Spanish or other language proficiency, community care management experience, and knowledge of Medicare/Medicaid are additional valued experiences for these positions.
