Meet Evercare Care Managers

Take a moment to get to know some of these amazing, caring individuals.

Jennifer, Care Manager

Years of social service work brought Jennifer to a Care Manger role with Evercare. Members are priority number one to Jennifer, and she dedicates herself to ensuring that they live their lives as healthily and independently as possible. She always introduces herself to families and takes time to explain Evercare's services. She can step in and resolve issues the family may have, and she encourages them to contact her with absolutely anything that can further the care of an individual.

"Navigating the health care system can be a very frustrating process for the elderly. For members without family to care for them, we have an important role in getting them services they need beyond medicine and the right doctors-things like meals, bathing, housekeeping and attendant care."

Yolanda, Care Manager

For the past five years, Yolanda has coordinated services for Evercare members living in an assisted living facility. It is very important to her that patients are at ease and she does everything to make sure all of their questions and issues are being addressed. Yolanda sets up appointments to meet them in person and, at the same time, maintains good relationships with the facility's staff to help see that everything is going smoothly so that patients don't have to worry.

"I view my job as going beyond coordinating medications and the right doctors. There are many other issues that are important to a patient's quality of life, and I work hard to make sure all their needs are being addressed on all fronts."
