We have independent contract opportunities available for clinicians looking for care management work on a contract basis through Evercare Connections, a division of UnitedHealth Group.

Evercare programs offer assessments, care coordination and ongoing care management and monitoring services, providing a systematic and holistic approach to understanding and assisting our caregivers and their families. As a Field Care Advocate, you will utilize your expertise in care management and home and community-based health care to assess clients in their homes, summarize findings, develop and implement plans of care, identify appropriate resources and provide ongoing care management and monitoring services.
