Evercare Care Managers are health and human services specialists who help families care for people who are ill, are older or have disabilities, while encouraging as much independence as possible. Our Care Managers not only coordinate care for the individual, but also provide support to families and loved ones.

Evercare Care Managers work in a variety of settings: in the community providing proactive care coordination to individuals and support to their families or in telephonic care management positions.
